Currently we have a temporary Full-Time opportunity for a Care Coordinator (CC).  The CC will functions as a member of the Health Link Team. 

The incumbent will support the development, implementation, monitoring and evaluation of programs and services.  This will include the interaction with the high users’ cohort, caregivers/families. The CC will also be responsible for the development and testing of the care coordination tools and processes.

The CC will:

  • Support the care team in developing care plans in concert with clients and caregiver/family. This support will ensure the involvement of the client in risk factor and health problem identification, goal setting, self-care strategies, and choices of interventions for disease treatment and prevention, and health promotion. The support will also include information regarding existing services, relevant clinical tools, problem solving and mechanisms for engaging various providers.
  • Facilitate, coordinate, educate and advocate for care and services.
  • Work in consultation to empower the client and their caregiver/family to the degree to which they are comfortable.
  • Provide focus in ensuring a standardized shared care approach through enablers such as client records, care coordination tool, guidelines, etc.
  • Participate in quality improvement initiatives, including collection of outcome data for the Health Link.
  • Focus on helping clients and their caregiver/family and the care team to manage the illness and condition, through the use of special techniques, personal experience, expertise and encouragement to assist in bringing about the appropriate behavioral changes.

Qualifications, Experience and Skills:

  • Clinical licensed professional in one of the following disciplines (NP, RN, RPN, Community Paramedic, Social Worker, Psychotherapy).
  • Active membership in related governing College or Association
  • Experience working in the field of mental health and addictions is preferred.
  • Professional experience in health care should include linkage with other community health agencies, needs assessment, case management and/or discharge planning.
  • Demonstrated understanding of the broader health care delivery system and available resources within the Champlain region.
  • Possess a background in performing a comprehensive health assessments with a major focus on related lifestyle, psychological, sociocultural, and environmental risk factors which may influence the client’s health status (and other determinants of health) and the ability to synthesize data from multiple sources to ensure the development of a comprehensive care plan.
  • Experience in working with the AROW target population (4 chronic conditions and vulnerable factors)
  • Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction.
  • Enhanced leadership, critical thinking, relationship development, organization, problem solving and communication skills.
  • Proficiency in the use of MS Office is essential
  • Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences.
  • Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a client.
  • Exhibit an ability to be open and non-judgmental, high empathy and compassion.
  • Current Ontario Driver’s License, access to a motor vehicle and comprehensive vehicle insurance.
  • Current Vulnerable Sector Criminal Reference Check satisfactory to the organization
